These forms allow you to make a statutory declaration under the Oaths Act 1867 in Queensland. A statutory declaration is a written statement that is declared in the presence of an authorised witness.
If the complaint relates to a vehicle offence and you were not the driver at the time, then you may complete a statutory declaration including the full name and current address of the person that was driving or in charge of the vehicle at the time.
